Diablo Immortal Madness Update Arrives With New Story, Gear, Gems, and Inferno Tiers
Sanctuary is sliding deeper into chaos as Diablo Immortal prepares its next major content drop. The Madness update pushes the Sharval storyline to its breaking point while stacking the game with new systems, bosses, gear, and long term progression changes.
What the Madness Update Brings to Diablo Immortal
The Madness update launches after scheduled server maintenance on December 16 and December 17, depending on region. Once servers return, the bulk of the update goes live, with a few features unlocking on specific dates shortly after.
This update is positioned as a full scale content wave, not a small patch.
The Epoch of Madness Story Reaches Its End
The main quest continues in the Sharval Wilds as Albrecht leads Shardborne legions through Entsteig. The final chapter introduces two new subzones.
Entsteig Bailey serves as a besieged outer courtyard of Sharval’s capital. The Barrow Tombs of Aughild explore the resting place of Entsteig’s fallen king and his warriors.
New bounties and Wanted Monsters arrive alongside these zones, fully rounding out Sharval as a complete farming area. This chapter closes the current Worldstone arc while clearly setting up what comes next in 2026.
Ancient Legendary Gear Enters the Game
Five Ancient Legendary chest items debut in this update. These items come with three fixed Magic Affixes and one random class Affix, making them more structured than standard Legendary gear.
Ancient Legendary crafting unlocks at Paragon 1200 with Inferno X. Players collect Ancient Artifacts from Helliquary Raids, daily activities, and dungeons. One hundred Artifacts can be crafted into a single Ancient Legendary at the Blacksmith.
Only two Ancient Legendary equipment slots can exist at any time. When a new slot is added in the future, the oldest slot will be removed.
Three New Legendary Gems
The update adds three Legendary Gems across different star tiers.
Fated Trail is a five star gem focused on brambles, Life scaling damage, armor shatter, and Immobilize effects.
Rampart Torch is a two star gem that triggers defensive shielding when attacked, reducing incoming damage and damage over time.
Flaystone is a one star gem that boosts damage and Critical Hit Chance against shielded enemies.
Each gem comes with its own resonance options tied to existing gems.
New Helliquary Bosses and Inferno Difficulties
Three new Helliquary bosses join the fight under the Fleshwarped banner.
Bloatmaw uses leaping attacks and toxic miasma to overwhelm battlefields.
Marrowmount charges through defenses as a siege focused beast.
Sundered Goliath carries Shardborne armies into combat while trampling enemies.
Alongside these bosses, Inferno X, XI, and XII difficulties are added, each with higher Combat Rating requirements and unique Helliquary rewards. Inferno IX requirements are also lowered.
Server Paragon levels increase to 1200, with massive experience bonuses applied below that threshold to speed progression.
Corrupted Heroes and Open World Threats
For a limited time, Corrupted Heroes appear across Sanctuary. These enemies mimic player style builds and combat behavior, creating PvP like encounters in PvE spaces.
Defeating your first Corrupted Hero each day guarantees a Legendary item. Additional kills can drop more rewards.
A special Cursed Heavy Ornate Chest event can also trigger multi hero encounters that scale with group Combat Rating.
Seasonal Events and Limited Time Modes
Brumaltine returns with daily tasks, milestone rewards, festive cards, and login bonuses.
Cutthroat Basin comes back as a PvP focused limited time mode from December 24 through January 7, offering flags, materials, and weekly challenges.
The Winds of Fortune event doubles drops across many activities during two separate windows in late December and early January.
New Sets, Legendaries, and Balance Changes
The Restitution in Blood set debuts with mechanics built around Fated Prey, execute style damage scaling, healing reduction, and self sustain on kills.
Every class receives new Legendary items that modify core skills, add mobility, improve crowd control, or push damage scaling in new directions.
Balance changes touch Barbarian, Demon Hunter, Blood Knight, and Tempest, focusing on shields, crowd control, Shock interactions, and survivability tuning.
System Updates and Quality of Life Improvements
Essence Mastery is removed and replaced by Mastery Bonuses that unlock automatically when skills reach max rank.
Cursed items are removed from drops. A new Cursed Paragon Specialization replaces the old system with focused passive nodes.
Cross server dungeon matchmaking enters beta in select regions.
Party Finder, chat tools, UI combat feedback, set crafting materials, and Helliquary Gauntlets all receive meaningful updates.
