Throbbing Gristle is a cursed item you get after defeating Guillotina in Act 1. Once you take it, every unit dies instantly when downed, including your own cats, and you must carry it to the Wall of Flesh in the Caves to finish it. If the cat holding it dies, the Gristle is gone for that run.

This is permanent sudden death mode. There is no undo.

You obtain Throbbing Gristle after clearing Guillotina’s first House fight in Act 1.

Do not overextend for risky kills. Focus fire enemies so they drop before they spike you. Use Tanks to control space and keep ranged units safe.
