That tradeoff means drinking it at the wrong time can actively make a chase worse, while using it during a safe window can give you a much cleaner reset before heading back into danger.

Drinking Beer in Halloween The Game lowers your Fear, but temporarily alters both your vision and controls.
That makes it a tradeoff item rather than a pure buff. You are getting Fear reduction in exchange for a short period where moving and seeing clearly can become more difficult.
I would therefore treat Beer as something you use after creating safety, not something you panic-drink while Michael Myers is already on top of you.

Drink Beer After You Get Away From Michael
The best time to drink Beer is when your Fear has become a problem but you are not currently being forced to make precise movements.
That could be after breaking line of sight, after getting out of a dangerous house, or while waiting somewhere relatively safe before heading toward your next objective.
The goal is to separate the downside from the danger. You want the Fear reduction immediately, but you want the temporary vision and control effects to happen during a moment where they cannot easily get you killed.
My preference would be to wait for that opening instead of drinking Beer the second Fear gets high. If the temporary control penalty overlaps with a chase, you can end up creating a worse problem than the one you were trying to fix.
Do Not Use Beer During an Active Chase
A chase is probably the worst possible time to drink Beer.
When Michael is close, you may need to make quick turns through doors, move around furniture, navigate houses, reach an escape, or react to a sudden change in direction. Temporarily altered controls and vision make all of that more difficult.
I would also avoid using Beer immediately before starting an interaction that you expect to finish under pressure. If you are about to work on an escape route or move Residents toward an exit, you generally want your movement to be as predictable as possible.

Beer Is Better as a Fear Reset Than an Emergency Item
The strongest way to think about Beer is as a reset tool between dangerous encounters.
Fear can become another problem layered on top of whatever Michael is already doing, so reducing it can absolutely be worth an inventory slot when your current match is getting messy. The mistake is expecting Beer to function like a direct defensive item.
It does not stun Michael, immediately heal damage, or create an escape route. It solves one specific problem: Fear.
That means its value rises when Fear itself is becoming the thing holding you back. If your health is low, you need an escape component, or Michael is actively pressuring you, another item may be more urgent.

Is Beer Worth Carrying?
Beer is worth carrying when Fear is one of the main things making your match harder and you expect to have a safe opportunity to drink it.
I would not automatically keep it over every other item, though.
An escape item may directly move you toward leaving the map. A healing item can deal with lost health. A defensive item can help during a confrontation with Michael. Beer gives you Fear control, but its usefulness depends on whether Fear is actually the problem you need to solve right now.
That makes Beer more valuable in a match where you have already been pressured repeatedly and less valuable when your bigger problem is simply finding the pieces needed to get Residents or yourself out.
